Responsibilities and Duties: 

 Round as a member of the Hematology Oncology team.
 Verify the accuracy and process all medication orders in pediatrics and adult patients receiving chemotherapy and supportive care in both the inpatient and outpatient setting. Verify the accuracy and process medication orders on select Internal Medicine Floors.
 Prevent, detect and resolve medication related problems and provide evidence based patient specific drug information and disease state management.
 Participate in clinical guideline development, implementation and assessment, disease management and drug use programs as identified and in coordination with the Clinical Coordinator.
 Identify process improvement initiative to avoid preventable errors and improve patient safety related outcomes.
 Participate in financial management initiatives for the chemotherapy.
 Demonstrate teamwork with the Pharmacy to ensure that medication and chemotherapy distribution is communicated and delivered timely, safely and effectively.
 Serve on hospital committees pertaining to Hematology/ Oncology
 Collaborate on the achievement of departmental goals and objectives.

 Provide staffing support and on call duties to the inpatient pharmacy on an as needed basis.
Specific Responsibilities and Duties:
 Round as a consistent member of the multidisciplinary team
 Develop a medication management routine:
o Assess the current medication management:
 Develop medication therapy goals
 Identify, prioritize, manage and prevent medication related problems (MRPs)
o Develop, implement and follow up on the medication plan
o Communicate with patient/health care professionals
o Document in patient charts when applicable
 Participate with the Clinical Coordinator and Pharmacy Department in implementing pharmacy initiatives.
 Follow a consistent method of evaluating/recommending the appropriate use of high alert medications such as pain medications.
 Assist in the management of side effects secondary to oncology treatment.
 Evaluate prospectively drug therapy for appropriate indications, dosage, drug interactions, and drug allergies.
 Monitor the patient’s pharmacotherapeutic regimen for effectiveness and adverse drug events and intervene accordingly.
 Evaluate parenteral nutrition in collaboration with the clinical dietician and recommend modifications as indicated to optimize nutritional regimen.
 Reconcile medications (prescription, OTC products and supplements) upon admission and discharge to determine if any findings may impact patient care.
 Counsel patients prior to receiving chemotherapy and on discharge medications as necessary.
 Provide specific pharmacokinetic monitoring when a targeted drug is prescribed.
 Develop, review, update and implement pharmacy protocols, policies, order forms and clinical guidelines
 Participate with the Clinical Coordinator, Clinical team and Pharmacy Department in evaluating and measuring the impact of newly implemented processes.
 Develop and maintain an Oncology Competency for pharmacy staff and technicians.
 Serve as a liaison with medical staff and clinical coordinator to educate health care professionals with regards to pharmacotherapy related issues.
• Serve as key drug information resource for medical, nursing and pharmacy staff, as related to oncology pharmacotherapy.
 Provide timely investigation and corrective action to incidents and adverse drug events.
 Document and report on clinical interventions and cost avoidance as part of the justification for the position/service.
 Collaborate with pharmacy, nursing, medical staff and the hospital administration on standards for accreditation
 Manage performance improvement activities and quality assurance for pharmacy-related issues in oncology.
 Develop process improvements to reduce drug errors and preventable ADEs.

• Evaluate how drug costs may be minimized through appropriate use and implementation of cost-optimization measures.
• Contribute to improving pharmacy information technology when applicable.
 Fill in for pharmacy staff when needed and address on-call duties.

 Perform data entry and assist in controlling the billing of daily surgeries done in OR and OH.
 Send on Daily basis the consignment orders required for scheduled surgeries to the Purchasing Department for processing.
 Assist in ordering on weekly and monthly basis from the Central store the needed non pharmaceutical stock items as per the Hospital policy.
 Receives Stock from central store and Check the quantity, quality and expiry date of all received items and report to central store any discrepancy or non-conformity.
 Store supplies on the assigned shelves.
 Avoid the shortage of stock by monitoring the issued items on the charging bills and do the necessary to replenish the missing ones on urgent basis.
 Provide the floor with any requested item, then prepare and send the return paper to the Data entry operator for processing.
